Employee Wellness

The City of Crossville is committed to employee health and helping employees develop healthy lifestyle habits.  This Wellness Program has been designed to motivate employees in reaching their healthy goals.

Any employee interested in participating in the program should print  the attached documents.  The waiver will need to be completed and submitted to the Human Resources Department.

Reimbursements for memberships, exams and weight loss will be done at the close of each quarter.  Turn all proof of exams, payment and weigh in at the Human Resources office.

Monetary Reimbursement Schedule:

Flu Shot

Weight Loss

Employee will weigh in each quarter with Human Resources. Initial weigh in will be January. Report to Human Resources to weigh in and have current weight documented. To receive monetary reimbursement employee must come by Human Resources in March, June, September and December. An activity log must be maintained and submitted each quarter to qualify for reimbursement.

Gym Memberships, fitness classes, 5k entry fees

50% reimbursement with paid fees/membership

Proof of payment along with membership description must be submitted each quarter for reimbursements. An activity log must be maintained and submitted each quarter to qualify for reimbursement.

Complete the registration and waiver and return to be eligible for weight loss and reimbursement incentives!
